Fee for service

Warrior Rx is a fee-for-service physical therapy clinic. This means we do not bill your insurance. You may be provided a Superbill at your request, which you may submit to your insurance for reimbursement under any out-of-network benefits.

WHY DON’T YOU ACCEPT MY INSURANCE?

At Warrior Rx, your care and time are our top priorities. With the quality of healthcare at an all time low and insurance co-pays at an all time high, the fee-for-service model allows us to provide you with the one-on-one specialized care focused on achieving your goals. The traditional insurance model often dictates the amount of time and type of care you receive to maximize a clinic’s reimbursement, which results in the use of passive modalities and support staff (technicians and aides). The fee-for-service model provides you a 55-minute visit, one-on-one with a Doctor of Physical Therapy determining the best care for you.

With fee-for-service there are no hidden fees. Payment is due at time of service.

DO YOU ACCEPT MEDICARE PATIENTS?

Unfortunately, due to medicare rules and regulations we currently cannot accept any patients who have medicare insurance.

CAN I USE MY HSA/FSA?

Yes.

DO I NEED A PHYSICIAN REFERRAL?

With fee-for-service, no physician’s referral or pre-authorization is required eliminating a delay in your care and allowing a quicker return to function and sport. If it is determined during assessment that medical evaluation is required your physical therapist will refer you to your physician.
